InSite Housing, Hospitality & Health Services
Vancouver, BC, Canada
Program Title: The Mentorship Program

The provision of quality services, care and choices is a large and growing challenge around the world. The Mentorship Program, developed by inSite in partnership with the British Columbia Interior Health Authority, supports, educates and mentors rural communities in the delivery of hospitality and personal care services. The ultimate goal is for the communities to be self sufficient in operating assisted living residences in their region. Having these projects within small and remote communities fulfills the provincial mandate of "Right Place" for eligible seniors. It also maximizes the use of scarce resources in cost effective ways.

Uniting Care Ageing South Eastern Region
Wollongong, NSW, Australia
Program Title: Improving End of Life Quality Care

The goal this programme is to improve the quality of life towards the end of life by reducing the use of unnecessary medications in recognition that quality care does not end as a person approaches death. There is a strong educational focus for the resident, the family and significant others. The program is managed by the nurse practitioner who works collaboratively with the attending medical officer. The reduction in the numbers and frequency of the medications taken by the resident lessens the physiological stress experienced and promotes a more peaceful time for the resident and family.
